Improving Adult Vaccination Practices Toolkit

This toolkit provides strategies and templates to help clinicians and other healthcare professionals reduce the risk of outbreaks, serious illness, hospitalization, death, and direct medical costs by increasing vaccination rates for influenza and pneumococcal disease in high-risk and older adults in Northeast Ohio.
